I see patients every day that not only have overweight or obesity but have also developed sequelae and comorbidities such as high blood pressure, cardiovascular disease, prediabetes and diabetes, high cholesterol, kidney problems, and more. Although I work hard to treat my patients through pharmacotherapy, lifestyle interventions, weight-loss surgery, and health education, I know that most will live a shortened life and be indefinitely reliant on medications such as insulin, statins, ACE inhibitors, and others.
Based on what I see every day, I know these conditions are all interrelated, but the traditional model of health care forces physicians to treat them individually. Many of my patients also see a cardiologist to manage their heart failure, an endocrinologist for their diabetes, and a nephrologist for their kidney disease. We need a comprehensive, multidisciplinary, revolutionary model for treating patients with this constellation of health problems, but even my specialty of obesity medicine is newly recognized by the American Medical Association, and fellowship opportunities in this field are scarce.
Obesity, arguably the biggest health crisis in the U.S., wasn't even recognized as a chronic disease until 2013.
I am frustrated with the current system. I am ready to embrace a better model of care.
